Soewadi, 1996 Psychiatric Intervention to Testicular Feminization Cases

 

During the life of mankind, there are many situation could exist. Among those situation are the disorders of sexual development. One of those sexual disorder is testicular feminization and its variants. The gonad in that condition is a testis, but the fetus is phenotypically female because a defect in the androgen receptor confers androgen insensitivity. Because the testis is normal, it secretes Mullerian inhibiting substance, and Mullerian regression occurs (regression of the anlage that would develop into uterus, the tube, and the ovary). The vagina ends blindly, and the presenting primary amenorrhea. Thelarche (onset of breast development) occurs at the normal time because the testis at puberty secretes testosterone, which is then aromatized to estradiol that stimulates the growth of breast tissue.

Orchiectomy (removal of the testis) is recommended after puberty to avoid the risk of gonadoblastoma unless there is partial androgen sensitivity, in which case it is performed earlier than puberty to prevent the pubertal development of hirsutism and partial masculinization. Vaginoplasty should be done latter.

Due to those condition of testicular feminization cases, psychiatric intervention should be done to the cases, even to their family in order they can face the stressor and to be realized.
